IMPORTANT
  • Include all relevant documentation (e.g., photographic evidence, bills and other documents supporting your claim. If you are unable to provide these documents at the time of filing your claim, they may be sent at a later date).
  • The form is provided solely to facilitate the process of filing a claim. The City is in no way responsible for how you fill it out.
  • It is your responsibility to ensure that the City of Brossard receives your claim notice by the deadline.
  • If you have an insurance policy covering your assets, we recommend that you inform your insurance company of any damages that you suffer.
Processing Your Request
  1. As soon as your claim is received, the City will open a file and investigate the circumstances surrounding the accident.
  2. You will receive a confirmation of receipt.
  3. Once the investigation has been conducted, you will receive a written notice by mail informing you of the City’s decision regarding the claim you submitted.
If you have already made the necessary repairs, please make sure you retain photographic evidence of the damage as well as all bills and other materials supporting your claim. These documents may be essential for settling your claim.
